Our Model, Placements & Support

We offer supported living accommodation for people with mental health problems and those who, as a consequence of their mental illness or personality disorder, have offended in the past, and/or those who present a potential to offend or pose a risk to themselves or others. Our home offers 24-hour high quality supportive living services which are homely and tailored to the individual needs. We understand the complexities of community-based support for those with forensic history and those discharged from secure settings and have offended in the past. Our team focus on promoting independence to all our clients while identifying and managing risks. We believe in a community-based support model that provides a “least restrictive” alternative.

Our model of Forensic Support

The support we give is carefully tailored to each individual, and it allows us to address specific offending behaviours and other possible risks and we implement positive behaviour support.

Aim

Our aim is to maintain and improve mental health, improve the quality of life of service users, and manage risk of re-offending. We strive to give individuals the opportunity to live as independently as possible with help from our staff when needed.

Our team have the experience, resilience and passion required to overcome challenges, break down barriers and form productive and positive relationships with the people we support.
We continually assess risk and implement supportive plans that promote independence, while also monitoring and managing identified risks.
Our clients make their own choices and lead their lives in the way they want to, whenever possible.

Life at Our Home

We work closely with local authorities and outside agencies such as forensic outreach services and probation services to ensure that the support we offer meets the needs of the individual and their history.
Individuals with a forensic background may require support to make positive choices that reduce their risk of relapsing or reoffending.
We work very closely with every individual, their families, their previous settings and any health professionals, to ensure that the risks are comprehensively understood to enable us to build a robust transition plan and care plans that meets their needs and manages risk.
All our clients have a tenancy agreement and are given the same rights as anyone living in their own accommodation.

Transitioning to our Home from Other Services

Life at Our Home

Our focus is on ensuring that people have the support they require to live a full and rewarding life. For individuals with a forensic background, our Home living services offer a structured environment designed to promote independence and manage risk. These may include:• Supporting you with developing your hobbies and interests.

• Improve daily living skills, including cooking, cleaning and shopping.
• Assisting in sourcing paid or voluntary employment opportunities.
• Re-engagement with education and learning.
• Supporting you to manage your personal care and medication.
• Maintaining appropriate behaviour and good communication.
• Building positive relationships.
• Socialising and getting involved in the community.
• Maintain a tenancy and deal with correspondence.
• Assistance in managing finances, including budgeting and benefits advice.
• Building confidence, resilience and self-esteem.

Forensic Placements

We support individuals conditionally discharged from secure units or prisons into our Home. With skilful support from our team, and working closely with the forensic outreach service, and probation services, It is our hope that we are able to move some into their own council flats and housing.
In our Home, every effort is made to create a warm and homely environment to all our clients. We strongly believe that supported housing, need not look and feel institutional. Our Home allows people to live in the accommodation as their own, where they feel safe in the knowledge that care or support is close at hand whenever they need help.
